Trapper John, M.D. sleduje doktora Johna „Trappera“ McIntyra 28 let poté, co odešel od 4077. mobilní polní chirurgické nemocnice (MASH) v korejské válce. Za tu dobu se postava výrazně zklidnila. Nejenže se naučila přestat bojovat se systémem, ale svým způsobem se jeho součástí stala — Trapper je teď primářem chirurgie v San Francisco Memorial Hospital. K pacientům chová obrovský soucit a kvůli nim často porušuje „zavedené nemocniční postupy“. Po boku mu pracuje ctižádostivý mladý lékař George Alonzo „Gonzo“ Gates. S Trapperem má hodně společného, protože i on sloužil v MASH (byť až za války ve Vietnamu). Jeho smysl pro humor a chuť do života připomínají Trapperova mladší léta. Gonzo v seriálu bydlí v obytném voze (přezdívaném „Titanic“) na nemocničním parkovišti.
